Home Visitation Services

Individual case management. Upon intake, each parent is matched with a Family Support Worker. Together they create an Individual Care Plan (ICP) to address the families short- and long-term needs (health care, housing, food, education, violence, abuse, job readiness, life skills, baby care, parenting skills, nutrition, career readiness, other) and linkages for support.

Regular visits

The family and FSW meet weekly and long-term (2-5 years) to implement the family's plan. The FSW helps young mothers and or fathers navigate services, provides support, and holds the youth accountable as she makes progress in healthcare, parenting, education, and career while transitioning to independent living.

working one-on-one

FSW also works one-on-one with families on life skills, positive parenting, and building strong parent-child relationships. Children are not only cared for through physical needs but mental, emotional, and spiritual relationships they form with parents and family members in everyday activities such
as reading a book, eating together, and giving hugs.
