Opening Snapshot — “Before”

When Destiny reached out to Healthy Babies Project, she was living in a tent city while pregnant with twins. She had no safe place to go and struggled to attend medical appointments while trying to escape an abusive partner.


The Challenge

Destiny faced barriers that no young mother should have to navigate alone. Homelessness, safety concerns, and inconsistent prenatal care put both her and her babies at risk. She knew she needed stability — and a team she could trust — to keep her twins safe and healthy.

The Support from HBP

HBP matched Destiny with a dedicated case manager who worked closely with her to establish consistent prenatal care, navigate safety planning, and access essential resources.

Over time, Destiny began to trust the process. For the first time, she had someone who checked in, showed up, and held her accountable with compassion.

Her case manager helped her secure temporary housing, complete the paperwork for the twins’ birth certificates, and apply for jobs — all while maintaining regular contact with her medical team.

A Turning Point

Destiny had a history of running away, but as she told us, she realized she couldn’t run away from twins. As she continued meeting with her case manager and taking each next step, she started seeing a future for herself and her babies that felt possible — and within reach.

Destiny begins a new chapter

A New Beginning

The twins were born healthy, though nearly three months early. They stayed in the hospital for several weeks, gaining strength. During that time, Destiny remained consistent, focused, and determined.

Two years later, her life looks completely different. Her twins are healthy and developmentally thriving. Destiny has stable employment. She has reconnected with her family and rebuilt positive relationships in her life.

And recently, she achieved a milestone she once thought impossible: she signed the lease on her very own apartment.
